What happens, in order

Book the consultation
Choose a hormone consultation time in our online booking system. You will get confirmation and lab instructions by email.

Lab work
The panel is collected so results are ready to discuss. Fasting or timing instructions are sent to you in advance when they apply.

One hour visit
Your provider reviews goals, history, medications and the panel with you, then explains which options are clinically reasonable and which are not.
- 04
Next steps
If a plan makes sense, you leave with it in writing along with monitoring intervals and a follow up interval. If it does not, we tell you that too.

Glacial Lakes Aesthetics & Wellness is led by Alexandra Malkuch, an Advanced Practice Registered Nurse who is double board certified as a Family Nurse Practitioner and a Certified Registered Nurse Anesthetist.
Plans here are assessed and recommended by a licensed clinician. They are not sold from a menu, and you will be told plainly when something is not a good fit for you.

What does the $499 include
The comprehensive lab panel and a one hour consultation with a licensed provider, including the review of your results. Any therapy, medication or follow up care that you and your provider decide on later is priced separately and discussed before it starts.
Do I need labs before I book
No. Lab work is part of this consultation. Bring any recent results you already have and your provider will read them alongside the new panel.
How long is the appointment
Plan on one hour for the consultation itself. Lab collection is a separate, short visit.
Are payment plans available
Yes. We offer payment plan options for care, and details are on our payment plans page or available from the front desk before you commit to anything.
Is there an age requirement
Patients must be 18 or older for hormone care. Anyone under 18 requires a parent or legal guardian present to consent.
Will I be prescribed hormones
Only if it is clinically appropriate for you. This is an assessment, not a prescription in advance. Some patients leave with a hormone plan, others with a different direction entirely.
